# Introduction to Aidin

> Solutions for Health Systems and Providers

## What we do

Aidin is a care transition platform built to eliminate the friction between discharge decisions and a patient's next site of care. Since 2011, we've connected hospitals, post-acute providers, outpatient clinics, social services, transportation partners, and payors on a single coordinated workflow — replacing phone calls, faxes, and disconnected spreadsheets with structured, real-time collaboration.

## How the platform works

Our core philosophy is simple: every stakeholder in a care transition — the discharging team, the receiving providers, the payor, the patient, and the family — needs to move together, not in sequence. Aidin unifies referrals, prior authorization (Auth), transportation, and more into one connected workflow so that each step triggers the next, rather than creating a new bottleneck.

Aidin operates as a two-sided marketplace: **Senders** (health systems) initiate referrals and authorizations, and **Receivers** (post-acute and community providers and payors) respond to them. Every connection and workflow aligns your healthcare ecosystem around shared goals and expectations for faster placements, better patient choice, and less manual coordination for care managers — for meaningful collaboration and measurable financial improvements in throughput and capacity.

## Solutions

* **Referrals** — Streamline discharge placement across SNFs, home health, DME, behavioral health, and community services
* **Auth** — Automate prior authorization workflows to eliminate delays at the payor boundary
* **Transport** — Coordinate patient transport as an integrated step in discharge, not an afterthought
* **IMM / MOON / Medicare form compliance and automation** — Ensure regulatory compliance at discharge with automated form generation, delivery, and tracking
* **Population health and care plans** — Extend care coordination beyond the acute episode with longitudinal care planning and population-level visibility

The guides below are organized around the role you play.
